First Session Policy

The following forms will be sent to you by Crescent and must be completed a minimum of 4 days prior to your first session:

  • Client History Form Client Information Form
  • Client Insurance Form (with pictures of insurance cards) 
  • Notice Of Privacy Practices 
  • Consent For Services 
  • Emergency & Other Contacts Form
  • Payment Authorization Form (for all non-Medicaid/direct pay clients)  

Cancellation Policy

Before your first session, please contact our main office by phone, email, or text if you need to cancel or reschedule. After you’ve started working with your therapist, you’ll reach out to them directly for any scheduling changes.

If you need to cancel, we ask for at least 24 hours’ notice. Appointments canceled with less than 24 hours’ notice, or appointments that are missed without notice, will incur a $75 fee.

We know life can be unpredictable. If something comes up, let us know as soon as possible so we can support you and keep your care on track.

Court & Legal Policy

Court & Legal Involvement

Our clinicians’ schedules are carefully reserved for client care. When a clinician is required to attend court, their day is blocked off in advance and regular client appointments are canceled to make space for the legal matter. Because those appointments cannot be rescheduled and the time has already been reserved, fees remain due even if the court changes or cancels its schedule within 5 days of the scheduled appearance.

Court & Legal Involvement

If a clinician is required to participate in legal proceedings, whether by client request, attorney request, or subpoena, the following terms apply:

Financial Responsibility

  • All court-related services are the responsibility of the client (or their legal representative/attorney) and cannot be billed to insurance.
  • This includes subpoenas, testimony, depositions, preparation, records review, written communication, and travel.

 

Rates & Minimums

  • Preparation, record review, and written communication: $200/hour (1-hour minimum)
  • Court appearances, depositions, or testimony: $200/hour
  • Travel fee: $50 in-county / $100 out-of-county

 

Advance Payment

  • A minimum of one hour ($200) plus travel fee ($50 or $100) is due 5 business days before the scheduled court date.
  • Additional hours will be billed following the appearance.

 

Non-Refundable Policy

  • Because clinicians must clear their schedules in advance, fees remain due once time has been reserved.
  • This applies even if the court reschedules, cancels, or does not ultimately utilize the clinician, including when the clinician is subpoenaed.

 

Failure to Pay

Unpaid balances will be treated as delinquent accounts, subject to collection procedures, and may result in suspension of therapeutic services until resolved.
